Because the load sits on your trunk, it makes everything heavier at once: push-ups, squats, dips, walking, stairs. That is both its strength and its risk, since the dose arrives everywhere simultaneously, including joints you did not intend to load.

Where it genuinely earns its place:

  • Progressive overload for bodyweight exercises once leverage has run out. This is the main reason to own one, and it is the weighted calisthenics route in its simplest form.
  • Making walking harder without going faster, which suits people whose joints prefer added load to added speed.
  • Raising the effort of anything you already do, including hill walking, step-ups and carries, in a way that is easy to measure and reverse.

What the evidence actually supports

Weighted vests have been marketed hard for bone density, particularly to postmenopausal women, and the picture is genuinely mixed rather than settled. Some earlier work found modest bone benefits from walking in a vest over a year, while the largest recent trial found no protective effect where it was most expected.

In the INVEST randomised trial, 150 older adults with obesity losing about 10% of their bodyweight over 12 months were assigned to weight loss alone, weight loss plus daily weighted vest use, or weight loss plus supervised resistance training. Daily vest use did not prevent the loss of hip bone mineral density that accompanies weight loss, and its effect on skeletal outcomes was broadly similar to resistance training (Beavers et al., 2025, JAMA Network Open).

The sensible reading: buy a vest to make your training harder, which it reliably does, and treat any bone-density benefit as uncertain rather than as the reason for the purchase. Adding load to exercise remains worthwhile in line with the NHS strength guidance, and if bone health is your specific concern, that is a conversation with your GP rather than a purchase decision.

The physio's cautions

  • Do not live in it. Wearing a vest all day for passive gains mostly delivers passive load to your neck, lower back and knees at a dose nobody programmed. Load is medicine at the right dose, and ten hours of wear is an exposure rather than a dose.
  • Running in a vest is a big step. Impact multiplies the load, and the shin and knee complaints follow. Earn it with months of loaded walking, then add minutes rather than kilograms.
  • Skip it if you have current back or neck pain. Axial load through an irritated spine rarely helps, and there are better ways to progress while it settles.
  • Take care with balance. A vest raises your centre of mass, which matters on stairs, uneven ground and single-leg work. Start on flat, predictable surfaces.
  • Not for children, and not during pregnancy without specific advice, for the same load-distribution reasons.
  • Watch for shoulder and neck rub. A vest that chafes gets worn less, and a vest that swings changes your technique. Fit matters more than brand.
